I found it if anyone is interested. I am guessing these were never installed and are probably NOS. I have no way to test them but the last set I sold worked for the guy that bought them.

I hope you didn't go to much out of your way.
I ended up buying that individual fader that John posted from Mackie instead because it was so inexpensive. I'll be soldering it in this weekend if I have the time.
If I have any issues with it though, or doesn't work out, Ill be knocking on your door again to offer to buy it off you if you don't already have it sold by then
Good luck on selling it though, I know there is probably a good demand.

No worries. I literally walked into the studio and saw it sitting on my parts shelf. Good luck with the fix.
